You may also designate an authorized agent to make a request on your behalf. To designate an authorized agent, you must provide LOTJD with written permission for the agent to make the request on your behalf or provide us with an executed power of attorney. You will also be required to submit both your and the authorized agent’s valid government-issued identification.
How to Appeal a Denial of a Request for Access, Correction, or Deletion of Personal Information
